The link should allow you to download it to your phone. All you need to do is create a new Note in the Notes app called Great Roads.
When you run the shortcut it will now ask if it is a Good Road or Bad Road, also with an option to Exit.When you click either option it will get your location and extract the street name and town/city and then add it to the notes along with a marker for Good Road or Bad Road.
You can then just look at the notes whenever you fancy.

Doesn't seem to work for me. I installed the shortcut and configured Scenic shortcut 1 for it. When I tap the shortcut 1 button in Scenic, the Shortcuts app comes forward (hiding Scenic) and the "Rate this road!" window opens. When I tap "Good," a notification (I think?) for the Notes app opens, showing the "Gread Roads" note but with no additional text. That's where my screen stays, the Shortcuts app forward, covered up on top by the Notes notification. I have to switch back to Scenic if I want to see it and continue to use it.
When I open the "Great Roads" note, I indeed find no additional text...
